I'm buying this half for nostalgia, half for what a big deal it is that FAKKU! has made the official handshake with Shiden Akira to make it officially available on our side of the Pacific, and half for how exponentially better it is than the janky fan-translation the likes of us made do with back in the day. That's three halves, but don't worry about it sssshhhhhhhhh

Is the achievement only gettable from buying the shirt? Also, the shirt, is like, the Mega Milk shirt and not the picture of her wearing it like in the thumbnail, right? I'm just having a hard time imagining people wearing unisex cut shirts saying, "Mega Milk." I can't stop smiling.

It's been so long since I've wanted to buy this official, uncensored release, to support the author. However, though there is a purchase price of $5.95 I believe, every time I click on it, I'm just brought back to the main page to read it. I'd like to buy and download it; I suppose that buying a t-shirt will also suffice, but only if someone could tell me the actual size that would be worn by the Akira character in the story. I have no intention of wearing the shirt myself, lol, but it would be a nice nostalgia collectable. Do I just need to buy a subscription to download a digital copy?

When I'm on this page: https://www.fakku.net/magazines/comic-megastore-2008-03 and I click on the purchase icon for $5.95, it just brings me back to the main page for that manga: https://www.fakku.net/hentai/tiny-boobs-giant-tits-history-english . I tried this using two different browsers, and I thought maybe it was my security settings, but I have everything displaying from Fakku unblocked. Do I instead need to buy a subscription?

Here (your 2nd link) is where you would purchase the manga.
Where it says "Start Reading" and "Download" there should be a Buy Now button.

@FAKKU QA: Thanks again for the helpful instructions! Turns out that there was just some weird anomaly going on where if I signed in and then tried to purchase, it didn't work; but if I add the item to cart first and then sign in, everything worked properly. Thank you providing such Godly work to the masses!
