About anime/manga clothing

Not sure if this is here, if isn't I won't protest if is moved.
For a long while I wondered about few things that I see in anime, anyone who lives in Japan, visit regularly or just know about could please answer this.
Why some miko outfits have the sleeves of the kimonos separated from the body and held around 2 inches away from the shoulders by red tread? And why other miko outfits have red ribbons in the sleeves? And lastly why others miko outfits are the 'plain simple' white kimono top without any kind of cut, ribbon or stuff like that? And please disconsider those clearly altered fetish miko outfits (shorter hakama, shorter kimono, clearly exposal of skin and interesting parts).
I am sure that as any otaku you all have seen more than one example of each of these.

Do women/girls actually use those clothes with separated sleeves hanging on their arms? You know the type, the one that leaves part of the shoulders and around half of the upper arm exposed. We see a lot of these in anime and manga.

I was almost forgetting, are skirts (uniforms in general, from school to work ones) used by girls/women in japan as short as we see in anime?

Well for now those are the questions that I have in anime related clothing, I won't even bring up hair styles on this.
