First, a guy cannot cum and piss at the same time. An erect penis is supposed to be set up so that urine cannot be let loose, only cum. So, no worries about a guy peeing in a girl's mouth if he's sporting a rod of steel.
As for a girl peeing while you're eating her out . . . I wouldn't break up with the girl, but I would quite possibly refrain from going down on her ever again.
