I <3 dakimakura covers
Amazon.JP has slightly expanded what you can order directly, but you're right that the vast majority of items are not available to overseas customers. So yeah, either proxy services or forwarding services would be the easiest. There are a lot of proxies out there, and I'm sure you can find one to suit your needs. I personally use either ShoppingMallJapan or SutoCorp.
I haven't set it up personally, but a forwarding service might be the way to go for Amazon purchases, especially if you already have an account set up. Big In Japan charge 500 yen per box to forward, plus whatever relevant fees/shipping is included. So just add their address to your Amazon.JP account, and you could likely have stuff shipped to them.
Also, on a semi-related note, be careful about dakimakura listings on Amazon. Not all, but a very good number of the listings there are bootlegs.

I was thinking of getting a keyboard from amazon.jp, because it's 20-30 dollars cheaper than the US one :(
seems like with the forwarding services, I'll have to pay for that difference with overseas shipping :(
and yeah, I've seen quite a few too good to be true deals around. thanks for the heads up anyway
