I find the Kamasutra to be a bit overated personally. It's fun for the shits and giggles of it all, but the basic positions (side-lying/spooning, doggy-style, cowgirl (or reverse), 69, and good ole traditional) can get the job done good and well.

Actually, this one is likely a lot more accurate than most others out there -- aside from all the standard positions, it also has sections on massage and non-sexual 'foreplay' (such as bathing each other).
As for the 'extreme' positions -- there aren't that many, but yeah, be careful. The ones that are a bit more adventurous are labeled (and explained) as such.
Above all -- know your limits, and SLOWLY expand them. Don't try to push your body past its breaking point, because you WILL regret it.

Another thing about the Kama Sutra is that it comes from India, and Indians have some of the smallest penises in the world (avg. 4-5 in.). And 2000 years ago 4 was probably big. So some of the positions in the Kama Sutra are designed for deep penetration but with today's guys sporting much bigger junk, those positions could be potentially painful.

To be honest, I think women don't give specific measurements for this because it's hard to determine a guy's girth just by looking at it.
