Usually its Lag, if someone on mic is really annoying I just mute.
Often I get matched up with "Veterans" who just destroy my team, this happens ALOT when I start to play Multiplayer (Lvl1)

I had 12 SC2 match where all my opponents were Diamond.
Great matchmaking, especially when I was only Silver.

In team-based games, those times when you get the pants on head retarded teams and just get your ass handed to you. No matter what you do, how many objectives you complete, people you take out. There's always more of the latter and they always have the manpower to take the objectives you took.
Also when the game itself becomes very stale, offering nothing new.

I've really come to hate games that make it impossible to play if you have had a late start in multiplayer, Like, Unlocks in weapons, armor, perks, whatever you need.
Like in Assassin's Creed, I've played the multiplayer ever since First Beta, and I loved it in its early days, where perks didn't automaticly = more kills... I really hate Revalations becuase of this, they've choose to make TINY maps, and if you'r not lvl 50, you've already lost, or dragged down the team.
Other than the ranking of some games, There also the "Hardcore" Compititive/PVP/DeathMatch player, The ones who ONLY play MP, Don't know why I hate them, but I do.

The first two thoughts that came up were campers and then spammers, but I already know that they're both fairly easy to work around if you have the patience. One thing that does bother me are the people that take advantages of glitches in a game just to get the upper-hand on their opponent. It's not only an example of bad sportsmanship, but it also takes the fun out of the game. Hell, that's one of the reasons why I stopped playing COD4. You literally couldn't find a match without someone glitching their way onto a rooftop, or under the damn map. At least when someone's spamming or camping, they're still adhering to how the game was meant to be played. To some extent, anyway.
Also, while it doesn't really annoy me, I can't bring myself to understand the reason behind t-bagging on online shooters. Sure, it's supposedly a sign of gaming dominance, but it's pretty much lost all meaning to me nowadays. It's even more questionable when the person doing it had just barely managed to take me down, even after getting the drop on me. This is taken even further when they aren't even that good. As in, they've only killed me about half the time that we've encountered each other, you know?

Couldn't agree with you more. Its impossible for me to play Halo online at all, especially since I've finally gotten 3 and Reach a month or two ago. After another month of trying, I just said screw it, and replayed the campaign. Pretty much when I play online on COD, I got the same problem, only add 10 year olds. I hate having to carry a team full of them, becuase of them whining....oh well its only a video game, so I don't get too mad, just enough to get irked.

I know of one such location, since I actally saw the guy "elevator" himself up.
The map was Strike btw.

After finally starting to burnout on Skyrim I've been trying out a few new games and I've had the same experience in a couple of them. I'm playing as a brand new player in a game that's been out a little while (but in one case only about three months), so I'm constantly kicked from the group for being low level. I kind of decided that I'm going to just stop playing the game because I haven't been able to stay in any groups.
There are two things that bother me about mics in online gaming. One is people who leave their mic on when they're talking to other people in the room with them. The other is people who hear a girl voice and get all weird about it.

People who won't shut the hell up. Nobody cares if your dinner is not done yet. This!!!!! better yet people period, they make or break your online experience if your with a good group then you can have loads of fun but if your with a bad group it can make you never want to play the game again

Kind of Important
A ray of Tsunlight.
People in general really. No matter what game it is, or how much I like it, someone at sometime will eventually piss me off enough to make me want to stop playing it for awhile.

@Vik:
In Halo, Rank is just there.. for cosmetic purposes.
The only unlocks you get there are just armor stuff, which is also just cosmetic.

Unsportsmanly behaviour. (such as in halo, being fuckwits, camping, etc. In forza, crashing into everybody on purpose. accidentally and saying sorry is fine)

Micspammers
Copious amounts of lag, usually caused by having too many particle effects on the map, too many gameplay-altering scripts running on the server, or a shitty server running the game
Players who take the game too seriously and rage constantly
Players who rub it in when they kill you, as if it's a huge achievement
When the game client suddenly crashes for no reason whatsoever
Banhammer trigger-happy server admins
