is the game play divided into selectable missions or is it stringed together like most valve games? and is this one of the games where a friend can jump in @ any time?

I'm not sure about the setup of the levels. I've heard that there are four campaigns which I think are made up of levels that go from the start to a few saferoom checkpoints then to a hold the area section while you wait for help to show up.
I imagine that it should be possible to jump in to one of the AI players mid-game, but I have no idea.

As far as I can tell, it doesn't always take place in a city. Like there's one level which is in a forest, I think.

Settings listed as a Hospital (the one I think I played at pax), a Forest ending in a Cornfield (the one Klorofolun is thinking of I think), a Murky Countryside, and an Airport.
And I was apparently I was wrong about difficulty levels.

We get the demo date before Halloween. And its made up of the first two levels of the Hospital level. No playable infected.
