This is about moments in games where you were fucking scared as if you actually felt threatened not just surprised. If you only got surprised, then that's nothing compared to actually getting scared from a video game. Of course I will also have to tell you what game moment scared the shit out of me.
It was in Bioshock, when I was walking around minding my own business killin' splicers that I let down my guard and thought I was alone again. So as I was looting the place I turned around and some statue bitch just appeared there. When I turned my back to it I felt the need to keep tabs on that shit so I looked again, but that shit was gone! At that moment I felt a little rattled and didn't know what to expect from the game so I kept going trying to be cautious when I heard a noise. As I turned around out of curiosity that bitch was standing about one foot away from me, just standing there looking as if she would've attacked me at any moment(her hand was drawn back as if she was gonna swing). I started freakin' out and the first thing I did was just stand still so that my camo would activate making me invisible(Yes, I was so scared I didn't want to move). That bitch started moving again since I was invisible to her and then I ghosted the bitch as payback. That wrench to the face had to hurt. Here's a video of one of those statue bitches I'm talking about so you know that they ghost around:

Well, L4D isn't really a scary game, but I have to admit, I got fucking shocked and scared at the same time one time. I was walking around hearing the witch music and her breathing, but didn't know where she was... until I literally opened the door on her! Scared the fucking crap of me and I quickly closed the door and ran over the cubicles for cover. This was in Dead Air, the Crane level on Advance. It was nothing like finding a Tank face to face suddenly; I seriously got the bejeezus scared out of me when I literally opened the door on the witch.

Two games cum to mind for me.
VtM: Bloodlines
There's a hunted hotel level early in the game and you have to find something in it. All the time one fucked up ghost is throwing shit at you (from pots to elevators) while a bloody spectre of his murdered wife shows up from time to time to scare the hell out of you warn you. Add that to rooms being set aflame in split seconds and clues of how the guy butchered his family here in the past and you're in for quite a scare.
FEAR
No particular level really - the game delivered from beginning to end. Whether it was glimpses of stealth assassins jumping into ventilation shafts only to attack you after 5 goddamn minutes of complete silence or Alma scaring the crap out of you while turning around to walk down a fucking normal ladder, I rarely felt relaxed or safe.

That ladder part really got me there, even though I was only watching my friend plat it...

Fatal Frame 3
When facing the boss in the 1st chapter, I was really caught off guard.
the was seating in the corner, face down. I was like "What Now?", then my friend said "Get close to her". So, with camera mode on, I approached her, then suddenly she jumps at you, and in a instance I press the shutter and caught her ghostly face.

Silent Hill 3
The Borely Haunted Mansion scared the shit out of me, after going through two rooms i reach one where i hear a rocking chair creeking but i can't see anything because of the camera angle, when the angle changes i see someone at the other end of the dark room sitting in that chair with his back towards me rocking back and forth, im 100% positive he's gonna turn around and lunge at me at some point, so i sneak around the room slow as hell to be prepared for his attack, then, out of nowhere, a shrieking corpse crashes down from the ceiling right in front of me, catching me totally off guard as i soley concentrated on the (dead) guy in the rocking chair.
As said, scared the shit out of me.

When i was about 5 or so playing zelda: a link to the past I was always scared to do this one room in the first dungeon where it was dark and had those fast moving statues with the big eye chasing me. Seems silly now. I've played a lot of games and seen a lot of horror movies since then and nothing really scares me anymore.

I'm not really a big fan of horror games so most of the times I stay away from that genre.
But In the case AVP2 I had to force myself through it since I was a big fan of both the movie series. So I decided to get myself a copy of it and started on the marine campaign.
And the first map still haunts my mind. Especially the part before you disable the security system and you make your way through the remains of the marines posted there before you. And also since you hadn't encountered any Aliens before this part you were expecting that it would jump one at you around every god damn corner you passed.

Half-Life - my first encounter with a barnacle. I'm standing there, listening to what that doc has to tell me when suddenly the ceiling would come crashing down and through all the rumble I see a barnacle lashing down and pulling that doc up. After about five seconds with a "WTF?" written on my face, and about the time blood and bones would start raining down, I would just quicksave & alt+f4.
Bioshock - the morgue scene at a pretty early point of the game, when you're walking slowly through water to where you're seeing a shadow from some doctor doing... stuff... being reflected on the wall, shortly before you get around the corner to wtfpwn his ass the lights would go out. My first reaction, aside from getting THE HELL OUT OF THE WATER (Which resulted in me running against the wall like an idiot until the lights would go back on.), was to switch from revolver to shotgun. You get around the corner and the only thing you see (Aside from his "experiment" he was working on.) would be one of those giant locker doors being shut down on you. I was freaking scared on my way back, but when he finally leaped out of one of the doors on the other side I'd put some shotgun shells strangely accurate between his eyes - that doesn't help the fact that I spent another three to four shots spreading barrage through the room in a "FUCK THIS SHIT!"-moment, though.

Xenogears, the part where you roam through the sewers to hunt down and kill a monster in there. It was like ten years ago and I was still fairly young. It scared the hell outta me. Its an rarity even among the rpgs. I actually jumped when I finally got to fight that monster. Super late at night XD

The first time I played through silent hill 3, perhaps the scariest moment in that game for me, was before the amusement park sequence that KLoWn showed. In my case, it was at the "alternate hospital", where you enter this room with a huge mirror. At first it seemed like an empty room...but then I couldn't leave...when I turned around and looked at the mirror....
Is it just me, or is it a trend in the SH games to make the hospital level the most fucked up of em all? In the first one, I remember that's where Lisa apparantly dies (or at least her face becomes covered in blood). In the 2nd, there was that sequence in the elevator where your radio was picking up a transmission from a game show..and pyramid head who had "apparently" killed Maria. In the 3rd, when you climb that ladder up before going into the "alternate" hospital...that was pretty fucked up...the whole walls smeared in that pink ooze (or whatever it was). In the 4th, you had a room where there was a big fucking head that would follow your movements with it's eyes...
