I've never really had any problems either, and I often make the "mistake" of keeping my discs inside my 360 when it is not in use. The only disc that may end up giving me problems is ironically MW2. It's a bit dirty and even a tad bit scratched up. I should probably clean it, and at some point the scratches might actually become prominent enough for the game to no longer function properly.

I keep mine in their original casing and I've had problems with cracks/scratches before. It happened when I was still using my old, fat PS3 and it seemed to damaged the blueray disc after long hours of nonestop use. The disc still runs but now it has a delay before starting up.

I generally keep my discs in their respective cases or in one of those special Disc containers/carriers that can carry up to several dozen discs at a single time.
I did cracked up a disc once or twice, but it was always on account with me being careless or forcibly pushing a disc into it's case.
