i have been wondering for a long time... but where do the characters from the hentai doujins come from a manga the game or are they made up?

All. Doujins are based off of anime, manga, and games. Anything that isn't based off of copyrighted material is called H-manga.

They just "pop" out, like an idea. Since Monster Hunter uses customizable charaters, armors and weaponry, they tweak some of the normal armors and girls into their likings and making them into a H-doujin.

Maybe it's like a popular original character. like the OS-tans, completely original from 2ch.
