reformat phone memory card

i cant reformat my phone's memory card for some reason, it says formatting failed or cant reformat......

I recently came apon the same problem. The micro sd card in my phone had become corrupded. After trying it in many devices (Nintendo 3DS and others) I thought it was a goner. I took it out of my phone and plugged it into a micro sd usb card reader and reformatted in on my computer. Then I put it back into my phone (HTC Incredible S) and reformatted it again. IT WORKED! I got my 16 gig micro sd card back. Give that a try...it worked for me...
PS...I had tried to reformat it with my phone...twice!...it failed both times.

i tried to connect the SD card using different readers, and its still the same, sadly idk how to turn off the read-only thing and the "help" page is too long and too many links

You said SD, the one you just posted is a micro SD.. Should have clarified that better.
And you don't use a a SD card with your micro sd inserted to connect it to a PC?

my bad >_< it was micro SD
i use micro SD USB, mp4 with micro sd card slot, (insert it on phone and plug in phone usb)to connect it to my PC

Hit Win+r(run command) -> compmgmt.msc -> Disk management(on the left sidebar) -> maximize window -> Scroll down to SD card letter -> printscreen and post screenshot.

You could try re-partioning it. Erase the current one and set up a new one which is what I did when my external hard-drive started to never show up on my or any computer.

Elevated CMD > diskpart > list disk > select disk # > clean > create partition primary > format fs=ntfs
(if NTFS doesn't work try FAT32)
(if NTFS doesn't work try FAT32)

If drive H is your card, then things are certainly weird. I've never seen an SD or uSD card reach the terabyte levels. Furthermore, windows cannot by default read RAW partitions, so you'll have to repartition like the two others said. From that image, right click in the "1335.87GB RAW" area->Delete Volume. Then right click->New Volume and follow the instructions to create a fat32 partition.

If the above doesn't work I would try using a friends computer with a different card reader to rule out anything. If that doesn't work you will probably need a replacement. It shouldn't show it having a terabyte of space. Something is wrong with the card's file table.

yeah, drive H is the SD card memory, i tried doing what you said, my PC just restarts
havent tried it on other PC, my friends think it got some virus or something

Try out Pyre's suggestion, partitioning through the command line. Then,
