Innovation doesn't happen overnight and in a time where nearly everything that can be made and every creative way of thinking that can be thought, in the majority has in fact already been done. People throw the word of creativity and innovation around like it's something that can be bought and sold on a store. Let me tell you this now, making something new and innovative is fucking hard as shit.
As an architecture student i literally know this fact more than anything else. Tell me, how many ways do you think you could rearrange rectangles and squares in a way that hasn't already been done in the million or so buildings and homes that are already on the Earth? This is the trend that liters the word of design in society today, nearly every type suffers this same sort of situation in their own way.
And also, you talk like the gaming industry is solely just limited to mainstream titles. Have you not heard of Minecraft? I think that's pretty innovative. What about indie games like Limbo and Bastion? And how about the culture phenomenon innovation that is Angry Birds? The video game industry is more than the traditional platform now. If you didn't know mobile/tablet games as an industry are worth roughly $34 billion, almost half of the total net worth of the video game industry. In the next decade it will easily overtake the traditional software games you're accustomed to to become the biggest entertainment industry in the world par sports.
Hmm. That's not exactly the point I as trying to make, and maybe I could have worded myself a little better, so allow me to try and rephrase myself;
You're right, there's only so much one can think of using the tools and ideas that they have at their disposal, and they can leave to the human imagination. There have been some great examples of people breaking the norm, and doing new things that have been quite... good.
However, the point I was trying to make is that in some cases, developers seem to take a step backwards, or make errors, that they should know better in making. However, when said developers make these mistakes, after being in an industry for so long, as young as the industry may still be, most people will write off these mistakes, when they should be noting them, and expressing their dislike for them, instead of simply writing them off. This is how we learn and grow as an industry, and if people aren't willing to do that, then we're going to continue to make the same mistakes or errors.
You're also forgetting that it is an industry.
Like it or not, companies exist to make money. While developers might care about innovation, the people paying them don't. Shareholders who own EA and Activison could care less about creativity. The reality is is that those re-run titles, like CoD and Madden are the cash grabbers. To put it straight, making new and creative games all the time is not economically viable because like you said, with innovation comes mistakes and errors, and a lot of them.
However with that money gained from re-run'd cash grabbers, companies like EA can push out some truly innovative games every year like Mirrors Edge.

...we *are* getting there. However it's going to be years until these techniques can be used in humans to replace entire organs (in the conventional sense of the name).
As fast as biological medical science is progressing i wouldn't be surprised if we had this ability in the next decade to be honest. To when it would be actually affordable to be of commercial use is another issue.
For example in 2008 three scientists won a nobel prize for taking a fluorescent gene from a jellyfish and implanting it into other organisms. In 2011, there are school communities you can find who order the needed parts online and teach you how to do it in less than an hour.
As for manipulating the human brain condition, watch this video and skip to 52:20, you might be surprised how far technology actually is right now.

That is misleading. A lot of that can be contributed to their culture as a whole. In a society where rape and molestation are regular if not mandatory occurrences, a stigma that money is everything despite Japan being in debt 220% of GDP, and a long history of suicide [kamikaze, seppuku etc] it's not surprising.
American society in the 1920's and Japan's society today are two very different things, so don't get too ahead of yourself yet.
