First of all, what have you played?

The only notable one I've completed is Clannad. Played a bunch of untranslated doujin games.

Only started getting into VNs not that long ago so I'm still a complete amateur. Started loads and then got caught up in studying.

I am so fucking jealous of Windows users and those who can read and speak Japanese.

I prefer the ones with a plot and storyline more, but I'll still play the straight up sex ones. Ones with a story are a lot better though.

Then you should definitely try Little Buster Ex. Fucking mindfuck near the end.

Also Da Capo (I, II & III), Utawarerumono, Tsukihime (Mystic Eyes of Death Perception is fucking badass), Saya no Uta (semi guro, not that extreme though), and virtually all Fakku top 10.

For lighter, slice of life plot, To Heart 2 Xrated tops my list (Sasara is just freaking HNNNNGGGGGG), Osananajimi wa Daitouryou, and Shuffle.

Been wanting to buy Da Capo I and II and pre-order Saya no Uta as they are English releases and I like physical copies of things. I downloaded the Utawarerumono rars but haven't installed yet. Thanks for the recommendations.
