Astigmatism is where your vision is generally blurry on all spectrum's from near to far. But since you say when you focus it get's better, I would think it's Presbyopia.
Presbyopia is a condition where with age, the eye exhibits a progressively diminished ability to focus on near objects.

Interesting... I would disagree with this due to me being 23, but I guess it might be possible. But before that I'd state that it's due to the amount of time, I used to use on a screen, and have recently fallen back into.

It can't hurt. I'm 25, was having headaches all the time, finally got my eyesight checked and learned I was slightly nearsighted. Now I wear glasses when I go out, never really have headaches anymore and it's like I'm seeing the road in HD now. lol

The best thing you can do is just go have your eyes checked although from what i ready i don't think there will be any problems.
I currently go blurry quite often where it seems my eye muscles relax and become unfocused and I'm on the pc from when i wake up to when i go bed, however i have been to the eye hospital and everything... i end up each time coming away being told i have 20/20 vision and such (I've had this since i was about 12 though)
Either way it can't hurt to have it checked.

yes, if you might need glassed go to the optometrist of eye doctor so you don't strain you eyes further and might get worse.
If that doesn't work you can always poke em out, they'll just grow back, I'm positive about it

Well let me diagnose u :P
U probably have astigmatism like me... it's not like u can't see close of afar ... u loose a percentage of the hole picture btw it grows worse ( i mean u see more like blur) with white light lamps and a when the sun sets (low light in general)...
if the doc prescribe u glasses u will only wear them when your eyes are tired but unlike other eye conditions the points don't change if u do not use your glasses...
As for the glasses !!! MAGANE IS GOOD !!! WEAR THEM TAKE 2 -3 PAIRS !!!
