Actually reply - I'm afraid of large crowds of people (but I've overcome to that to a degree). I'm not really afraid of one specific thing.

Lobster. I fucking hate lobsters. I had a nightmare the other week that people where throwing lobsters at me. In my sleep I got out of bed and started running across the room. Have you ever woke up in a panic, while already running? It's weird man.....

Spiders, small once, especially if there is many of them.
I think if i ever saw 1m or higher sized spiders i would not be afraid of them as much as 1m sized pile of small spiders.
Every other creature that's mythological or not is no problem, if it bleeds it dies, if it dies i will kill it, cook it, eat it.

Myself, serious answer. The inner workings of the mind, the self I don't know of, to be precise. After hearing about Charles Whitman, and having a sensible understanding of fear... I fear the actions of the self I can't control.
As much as humans scare and infuriate me because, I also find that to be one of the things most interesting about them.
I have been, with little progress, trying to teach myself to lucid dream, one of the warnings I received was to never look into a mirror. You will view yourself as how you truly see yourself (which is rarely positive). The shock can apparently knock you back into dreaming or mostly likely into a nightmare, from my understanding. So absentmindedly I sometimes become spooked of mirrors.
Also, spiders... The idea of something so small causing such a painful and fatal death... I'm ok with snakes for some reason though. Arachnophobia, even the way they move gives me shivers.

I would say spiders and centipedes but I don't consider them sick, fucked up creatures. I assume that by sick and fucked up, the OP is either making use of an hyperbole or refering to fictional monsters. If it is the former, I believe my answer fit. If it is the latter, I wouldn't have an answer. The fear of having a spider crawling on my neck is much more terrifying than a drawing or a bad movie.
