I remember reading this. To me, there seems to be more evidence that he preferred the game world and fell in love with a character from that game, then began to accept it as reality.
There's one major detail you've missed out; at the end of Chapter 6, the guy was sitting at a computer and used it to fall back into a coma as soon as he starts the game, leaving behind a limp body. I think that's enough evidence to prove that he was pining for a game character and that the blonde Yuka was telling the truth. In fact, she knew a fuckload about him that he didn't even remember (suffering from Paramnesia) and even had a scrap book of the both of them when they were little.
When he fell into a coma from the reality the dark-haired Yuka was in, there was no evidence that he remained there. In fact, as soon as he hit the Enter button on his laptop, he switched realities and we don't know what happened to his unconscious body afterwards, and when he woke up into the blonde Yuka's reality, he was in bed. Coma-stricken people tend to remain in their beds. When he fell back into a coma from the reality the blonde Yuka was in, he was motionless and time didn't just stop because he left because remember, games stop when players quit or leave. But it didn't, after he'd gone, the blonde Yuka stood behind his body and cried for him. There was no other time in the 6 short chapters that has happened.

i want to write a reply but your siggy really distract me lol the pain, anyway back to topic.
We can say that there are major rules that are shown thru 6 chapter
1. He always start with sleeping and the girl always beside/close to him when he wake up.
2. aside from 2 yukas, it is safe to conclude that the rest of the girls are connected in same world, this prove is supported by chapter where yandere girl and school nurse (chpt 1 and 2)
3. Assuming that this game always have him being with any girls and its mutual love without any rival, chpt 1 nurse X PC , chpt 2 yandere X PC, and so on the only girls that broke this is dark haired yuka
4. About your theory where his body in "so called real world" are never explained he is living alone right so its possible and the last scene follows rule number 1 where the PC woke beside dark haired yuka waking him up.
5. The MC body in game world can get harmed or die this one proved by chapter 2 and chapter 6 i believe but again there are no clear relation between other girls and yukas.

The reason why D.I.K. is one of the best hentai ever, the plot not only fucked the readers, their minds as well. You see, D.I.K. revolves around a normal, mmmm, eroge gamer, or was it visual novel? Whatever. But, D.I.K. is very interesting, per se, the guy, actually had a conflict w/ the two worlds he's living in, I think he did not, take note, did not fell in love w/ the blonde girl, I think he's in love w/ his neighbor(?), remember the part where he fucked the girl so harshly that it almost looked like rape, and the girl was crying like crazy. Later on, he realizes that the girl he likes is already having an affair, or something like that, nevertheless, it's narrating the life of a normal, ero-gamer, D.I.K. not only possesses it's fucking class points, and it's mindfucking abilities, but also the painful reality behind playing eroge/VN and it's psychological and social effects to an individual.
I'm tell ya, I've been in the exact same case, w/ this guy
