Most archive managers are capable of extracting IMG files. WinRAR is a good choice if you're using Windows and PeaZip is good if you're using Linux and prefer a GUI client. I'm not familiar with OSX applications so you'll have to ask someone else.From what I imagine, you're looking to install whatever is contained within the image rather than extract it to your drive. Daemon Tools Lite is a program for Windows that is capable of creating a virtual CD/DVD drive on your computer and mounting various image files. Once the image is mounted, if it includes autorun information, you should see an installation menu/prompt appear. If it doesn't include autorun info, you'll need to navigate to the virtual drive and explore it's contents, usually the installation can be started via a file named setup.* (where * is the file extension, often exe or msi in the case of Windows). If you need more in-depth instructions there are plenty of guides on how to use Daemon Tools on the web.
If you're using Linux, this guide will tell you how to mount it, no extra applications are needed. Though the instructions are written for .iso files, they apply to almost any other related format.
