Aliens vs Predator 2, playing as Marine. That fucking movement detector starts beeping.
Dino Crisis, there is a room with a dead scientist in it were one wall is just a big window, suddenly T-rex head pops in. Scared the crap outta me when l was a kid.
Much of Dead Space, but one time one of the little fuckers jumped out of a toilet and scared me a fuck load.

Okay, so, once, on Halo 3, I was playing the level The Covenant, and this Brute set himself, and his own allies, on fire with an Incendiary Grenade. My brother and I made a running gag of it, with the quote, "Look, it's the Arbiter! I'm going to set my friends on fire!"
