I just finished Blood C. At the beginning it was pretty tame. But the last couple of episodes have like a shit load of blood and gore. It actually slightly disturbed me. So Blood C is definitely a top contender.

Probably Hellsing Ultimate would have to be top for me. I have yet to watch this Elfen Lied anime though, currently on my plan to watch list. Other anime in regards to this thread though, would have to be already mentioned HOTD and Bokusatsu along with Bacanno with what I've watched.
