Since some people don't know what NTR is, it's where the main character's love interest or some one important to them is either seduced or coerced by some one else and may eventually end up cheating or betraying the protagonist.
Example: Protagonist is engaged to a woman named Sakura. Sakura meets someone - lets call him "Jun" - at the protagonist's work site on her way to bring him lunch. Sakura becomes interested in Jun for whatever reason - maybe because he is lonely and she feels bad for him. Protagonist is unaware of anything. Sakura then bumps into Jun in the mall, Jun then forcibly takes Sakura into a dressing room. They have sex - sex so good that Sakura now visits Jun constantly and proceeds to have even more sex. The protagonist finds out and spies on them during their encounters all while plotting his revenge. Protagonist either kills Jun or Sakura, or maybe both, or maybe instead decides to have sex with Sakura's little sister or Jun's daughter.
You can replace Sakura becoming interested in Jun with "Sakura being in debt to Jun" or "Sakura being blackmailed by Jun" or even "Sakura being raped by Jun and too ashamed to tell anyone."

Wasn't the most common scenario with a pussy main character like Shinji Ikari and doesn't do anything and even cry when he sees his girl being raped and enjoying it?

I dunno - I don't like NTR at all. I know that sometimes it's like a Schadenfreude thing, the viewer likes seeing the main character suffer. Or some people like it because it usually contains rape. I however think its just a sad story, especially when the main character just takes it like a loser.

Most of what is classified as NTR is not really NTR.
NTR is suppose to be from the main character's point of view. Basically put, if you were playing a Love Sim, it would be the guy you'd be controlling. Except instead of you getting the girl you would be experiencing the girl being stolen away from you while you watched.
Which is why it would create those emotions because most people subplant themselves as the main character (the point in them usually being ambiguous like that) and thus would feel it happening to them.
When it comes to manga. Most of the NTR isn't really because it's not usually the main character who is getting cockolded, it's someone else. Not to mention that the characters are rarely ambiguous and thus we tend to separate ourselves from them.

I was actually at an animation office yesterday and that was on the storyboard. It has a semi, slight little twist but eh, it still was a lame hentai story.

Thats an interesting point. I see the correlation to and I don't think it is only a coincidence. However, I have no idea why this is. Perhaps it is only because companies with the resources to put out niche stories like this are going to animate such things. A smaller company with less talented artist are usually going to stay vanilla.
