I've never cared much about the genitals in hentai anyway. If they started censoring boobs then I'd say forget it.
The variable degree of censorship in h-manga appears to be a decision of the publisher. Some of them seem to try to test the water a little more, but others play it really safe. It's probably similar to the porn business in the US where they operate off of a lot of unwritten rules because the actual laws are very vague and nobody wants to be that one company that stuck its neck out and got destroyed by the feds.

I guess that's true in some cases, but I've seen a lot of cosplay porn that mosaics the girl's face out. Maybe their faces were too different from the characters', and they didn't want to screw up the fantasy; I dunno.

Naw, I'm not referring to anonymity mosaics, I'm referring to hentai where the mosaics just go overboard.

It really is ironic how a culture with so much sex in it (like most other cultures) censor pornography. Luckily more series have minor mosaics and some manga are creative. Like Gunma's mag releases white out the tip of the penis so the shape is still there. But the tanks have like no censoring, I don't know if its decensored by the scanlator or not though.
Either way, this is irony at it's best and I am amazed when people ask for an uncensored version of something on torrent sites like BakaBT.

Ah, the lazy censoring. I'd forgotten about that, honestly. Yes, that does seem to be a case where they just plain didn't do the work.
I imagine it's because of the budget, though. Few animated ero series get enough budget to have that much attention paid to them.
As for the tankoubon, yeah, it's common practice to tone down the censorship for those. It's still there, of course, but much less noticeable. The black bars become smaller and transparent in the tank of Honey Blonde compared to the Comic Megastore version, for example.
They get away with this because the magazines (e.g. Comic Megastore, Kairakuten, etc.) can be found on magazine stands in out-of-the-way places, accessible to the public. The tankoubon have to be ordered specially, either online or through catalogs, so they're kept out of the public eye.

One ero-anime which censorship pleasantly surprised me was Natsu Mushi.
I was like, holy shit normally companies censor the crap out of it (see screencaps above), but they really did a good job on this one.

Seems like the coloring is a bit off on the hand, though, but I think it has something to do with how they went about censoring it (censor the whole thing, then put the hand above it again in another layer perhaps?)

1) Censorship is a part of the Japanese constitution.
2) While not strictly enforced, most hentai and porn in japan is censored to avoid legal issues. It's easier for an artist to just drop a black bar over a guys penis than to have a run in with a bored lawyer.
3) Ending censorship would require a change to the Japanese constitution, and Japanese political officials are in no hurry to get rid of it for the sake of better porn. Not a very wise career move.
tl;dr - There's an unspoken agreement that if these artists just drop a token censor over the naughty bits, the law won't give them a second glance.

In Japan it is illegal to show graphic depictions of the male and female genitalia in print or video under any circumstances unless for medical or educational purposes. So they censor it and wala! They can get away with anything. Funny thing is, it is illegal to show a full dick or vagina but they just put a black strip or bar over it and it's good. Still don't know why though...

They don't like the law, they know they can get away with doing virtually nothing to comply, so they go as far as they can while still being kinda-sorta able to say, "Hey, we censored it."
The industry has its own controls on things.
The more "mainstream" a publication - i.e. the more likely one is to find it on the stands close to more "respectable" mags, or the more likely it is that children can get hold of a copy - the heavier the censoring. You'll find complete whiting-out of genitalia there.
Conversely, some publications are only allowed to be sold in certain specific locations which aren't open to under-18 customers. These get away with tiny black bars.
Then there's the publishings that are sold direct to the customer. These are the ones you find with a transparent censor bar, which doesn't really obscure anything at all.

It's come to a point that this censor is almost like a trademark in hentais, maybe it was more strict in the past and they would just keep doing it without any good reason, what would people say if we started to do uncensored hentai? For example what if they would stop making openings for animes. Sure they could tell themselves "most people will just skip it, it's almost a waste of (insert whatever they could waste on making an opening that nobody would listen) so they would end up not making openings anymore. What would people say? Hey you,ve always been making openings, there are people enjoying them, why would you stop making them now?
That would probably lead to riots (mostly in japan) revolutions, urban warfares, governments would fall and then... world domination, most likely.

The best solutions is just to make both of them, a censured version and a non-censured version. So for those that think it is offending to see a dick or a vagina than take the censored version, but I don't care seeing a wet vagina being penetrated by a man. When I am watching a hentai movie i am looking most of the time at that part. If it's censured, well it is just killing the moment...
