Its hard to say what the saddest piece of music I've ever heard is, most of the songs that I find exceptionally sad are because they remind me of a story/character I really cared about, as well as being a sad piece of music.
For example, these two songs from Mirai Nikki, which is my favorite anime, featuring a fictional character I have an unhealthy obsession with.
Or this song from Final Fantasy 10, my favorite video game, and one of the things that bassically defined my childhood. Balled my eyes out the first time I finished that game.
