No, she didn't at all. Kotonoha was too timid to even touch Makoto before he lusted after Sekai; Kotonoha started to become desperate after Makoto started screwing Sekai everyday, but he pretty much ignored Kotonoha and spent less and less time with her. The one and only time she had sex was with Taisuke, and she was thinking about Makoto the whole time while doing it too.

Similarly, Sekai had sex only with Makoto, but Makoto had sex with all the girls. His death is divine vengeance, I tell ya.

It's more than that actually. Kotonoha suffered such an emotional blow when she learned Makoto went to the bonfire dance with Sekai that she lost all will to do anything, including stopping Taisuke from having sex with her. So, I think it still qualifies as rape.

They're not really doubled. If you look carefully, you'll see that one of each pair is the Japanese raw and the other is the English-translated version.

3 years ago, Haruhi caused some strange thing to happen to the universe, triggered by her realisation she was just another drop in a vast ocean. She didn't simultaneously destroy an old universe and create the current one back then, but she merely wished to see things out of the ordinary, hence the creation of closed spaces, time travelers, espers, and aliens. I would think that Haruhi would be somewhat aware of her own powers if she somehow destroyed the old universe and recreated it then.

If that is true, Haruhi is still a first year high school student. In Japan, most first year high-school students are 16 years old. I suppose 15 would be an acceptable answer too, but 16 should be correct.
