                        I will usually tip, since it's customary and something of good manners.
Unless the service is really shitty, which does happen quite often where I live, unfortunately.
No tip for waiters that treat customers like sack of potatoes or dog shit.
Otherwise, yes, I tip.

                        Not customary over here, and most people would actually think you're weird if you do this, so usually I don't. The one and only place I've seen here that actually tries to introduce this custom always gets a tip from me, though.

                        I usually leave between a 10 - 20% tip.  It depends on the quality of the service.  What I don't like is when restaurants automatically include a 20% tip in the bill.  It makes the service think they don't have to work as hard, and then the quality of my meal degrades.

                        I tip waiters/waitresses when in the US or countries where the tip isn't included in the bill.  In general, they get lousy wages with the understanding that patrons are supposed to tip unless there is some fault with the service.  My sis works at a restaurant for (officially) min wage but all the servers are told to expect to make about 15/hr because of tips.  I generally tip 15% rounded up to the nearest whole dollar.

Usually, many of these people get a share of the tips, not just waiters/waitresses. Not sure about cooks(they often get better wages), but bussers and hosts/hostesses usually get a share.

                        I always tip the customary 10-20% depending on how good the service was. But if the service is really and I mean really crappy then I won't tip, but this has only happened to me once.

I'm playing world of warcraft... So there's Whitecrayon spamming trade chat (again) for his jewelcrafting profession. I had 3 green gems in my inventory so I whispered him, here goes the conversation:
Me : Hey so... can you cut my 3 gems?
Whitecrayon : Sure what do you need?
Me : Just a few bloodstone, I already have the mats
Whitecrayon : Hmm how many of them?
Me: Three.
Whitecrayon : How much will you tip?
Me : ... ehhh 5 gold?
Whitecrayon : no thanks
I tried to whisper him again, but he /ignore 'd me... I won't tip anyone ever again
