-Yumeria had a shitty dub. But I liked the show.
-I wish F-Zero: GP Legends had a better Time Slot when I was a younger.
-Kirby: Right Back at Ya is a classic.
-Sonic X was in hindsight pretty terrible. But it was pretty cool at the time.
-Viewtiful Joe went perfectly, in my opinion.
-And To Heart 2 was adorable.

Katawa Shoujou is made by peeps around the world, scripted in English first, and will give you feels up the ass. A fantastic VA for free that changes your outlook on life, if you get the GOOD ending(Which is totally easy!).

Ah, i sometimes stray into that area but then i remember to actually care about things that aren't terible. They're sometimes good but not the best, and the bad ones are worse then the worst "Anime".

Either I didn't look far in the comment section or someone forgot about (*Cough*) Devil May Cry. I am entirely sure that the game and the anime series was amazing, hopefully I'm not the only one.

Not really any that have caught my eye, due to the fact that the ones I know of are obviously cash cows. (Pokemon, Halo, One time Mass Effect Movie, etc.)

Like any other kind of adaptation they are hit or miss.
I do like it when they try though since they can be awesome when done well. But like with manga or novel adaptations they can have problems and frustrate when they don't get it right.

I wouldn't count visual novels.
But most anime adaptions I've seen of games (like Devil May Cry) usually were pretty meh.

I have seen is Persona 4 The Animation which I thought was really good and the Devil May Cry anime which was interesting but really enjoyable for some reason. I'm looking forward to seeing the Persona 3 movie when it comes out.

I refuse to count anime based on VNs, both because VNs are not games (they're visual novels, there is a difference) and Shuffle! sucked balls. I am a huge VN reader, but calling them games is really stretching the word.
On the topic, Kirby made a great anime and so did F-Zero GP Legend. The Megaman NT warrior series was pretty cool too. I think the way to make a good video-game into a good anime is by making it a separate entity: That way it doesn't need to follow established canon and can be its own universe with similar characteristics.
You distance it enough to avoid enraging fanboys due to inaccuracy, yet you keep it similar to avoid pissing people off due to it not being close enough to the original.

I've never watched anime that I knew was based on a game. If I come across some random anime that gets my attention - I watch it, and read what it's based on and other stuff about it only after I'm done watching. Though usually the answer why the anime I just watched wasn't too good, was because it was based on a game.

Tales series, is not really that good overall, the game was good, but the animated project, is somewhat lacking. A lot has been edited, which could add to the OOMPH factor of the anime, but they did not add it. however, some games like
FATE Zero, FSN,Unlimited Blade works and Eiyuu Densetsu was particularly good.
they had a very good introduction, very good animation scenes, and the best of all, they had captured the most epic moments in the game.
Blazblue will be having an anime sometime this year, they had a good plot and good scenes, and is widely recognized in japan as an Arcade Game.
Now, they just have to be sure that they maintain that status and make sure that they don't blow it.
There were like 5 artes used in the entire series, they skipped out on the scenes where Lloyd fights Kratos and inherits the Material Blades and he never used Falcon's Crest

Disgaea, Final Fantasy VII movies/shorts, Persona, Shuffle!, Green Green, Street Fighter II: The Animated Movie, Street Fighter II V, Street Fighter Alpha, Street Fighter Alpha: Generations, Street Fighter IV, Darkstalkers, .Hack//sign, Pokemon (Season 1-5, Gen 1-2), Digimon Adventure I and II (didn't care for the Tamers and what ever came after that), Halo Legends, Clannad, Comic Party, Popotan, and one of my favorite Hentai series Words Worth are all good Anime/Movie/Hentai based off a Video Game.
